Everywhere, I say!  If you consider the possibility that the human race has always been doing iterative development, what is revealed is the need to write our acceptance tests first, shorten our build cycle by doing less, build more frequently and pay more attention to measurement / learning.   This requires the ability to look, learn and laugh at ons’s self as often as possible.  The laughing moves us forward :)
